Insurances: We are In-Network with many insurances not listed below so please call and confirm with your insurance if you insurance is in-network with our clinic. currently we are in network with  Medicare,    Blue Cross Blue Shield and Anthem,  United Health, BCN Humana, Aetna and Aetna Better Health,  Meridian,  Cigna,  Maritian, Tricare, Borgess Health Plans including Smart Health Plan

Co-Pays:  Deductables and Co-pays are required on the day of service, we accept cash, checks and major credit cards.

If you do not have an insurance: New patients with minor acute visit, fee is $85, and will be $77 for acute visits thereafter. (these are discounted cash rates as it saves us lengthy and expensive billing process)

Refills: Refills are electrtonically sent to your pharmacy during your office visits and enough refills will be given to satisfy until next scheduled appointments. 

Appointments: All patients with chronic conditions are asked to make followup appointments after their visit with a doctor. Acute illness visits usually do not need appointments, we do not combine acute visit with regular appointments.

We send text / email reminder one week prior and again 2 days prior to appointment.

First time in the clinic: We ask you to come at least 15 minutes before appointment time to fill up paper work. Please bring all medications you are taking along with I.D and insurance cards. and don't forget to ask our recepcionist to sign you up for electronic medical access of your chart.

Covid-19 Update: Masks are required to enter into clinic, if you have fever, cough, shortness of breath, change in smell sensation or cold symptoms then you must call our clinic before entering.

Labs and Radiology: We will give you order slip to get labs or x-rays done from laboratory of your choice, Currently majority of our patients are doing labs at Borgess Hospital across the street or at Quest diagnostic.

For Employers: Please contact clinic to establish your requirement for Pre-employement / ppd/ drug screen/ flu vaccinations or other servises at competative rates.
